Medical ID Cards

Imagine360 will send your medical ID card in the mail after you enroll. If you have not received your card or have lost it, you can access a digital copy at miBenefits.imagine360.com.

Visit imagine360.com/providersearch to see if your current providers accept Imagine360 and search for other in-network providers. Enter code: OHNVR, or GCRTD (for those who live in New York or New Jersey). You can also complete and submit the Provider Verification Form.

MEDICAL AND PHARMACY

Imagine360 Medical Plans

$5,000

HDHP with HSA

This plan offers a $5,000 individual and $10,000 family deductible. With this plan, you will pay less in monthly premiums, and you will be able to open a Health Savings Account which allows you to save pre-tax funds for out-of-pocket costs.

$3,400

HDHP with HSA

This plan offers a $3,400 individual and $6,800 family deductible. With this plan, you will pay less in monthly premiums, and you will be able to open a Health Savings Account which allows you to save pre-tax funds for out-of-pocket costs.

$4,000

Deductible PPO Plan

This plan includes a $4,000 individual and $8,000 family deductible. This plan will have higher premiums, but you will pay less for doctor’s visits by paying a set copay.

$1,000

Deductible PPO Plan

This plan includes a $1,000 individual and $2,000 family deductible. This plan will have the highest premiums, but you will have lower copays and a lower out-of-pocket maximum.

When you’re enrolled in an Imagine360 medical plan, you have access to a network of pharmacies through VeracityRx that provides prescription drugs and related supplies at discounted rates.

  • Oral exams and cleanings are covered 100% 

  • Yearly deductible of $50 individual/$150 family

  • Annual Benefit Maximum of $1,000

  • Orthodontic Lifetime Maximum of $1,000

Delta Dental of Kentucky – High PPO Plan

  • Oral exams and cleanings are covered 100%

  • Yearly deductible of $50 individual/$150 family

  • Annual Benefit Maximum of $1,500

  • Orthodontic Lifetime Maximum of $1,500

Delta Vision Plan with the VSP Choice Network

  • $10 copay for in-network eye exams and $15 copay for lenses every 12 months

  • $0 copay and $130 allowance for frames every 12 months or

  • $60 copay and $130 allowance for contact lenses every 12 months

Your Employee Assistance Program (EAP) is a free benefit available to you and your family, offering resources to support your mental well-being. As part of this program, you have access to up to five counseling sessions per member, per issue each year at no cost.

Health Savings Account (HSA)

A Health Savings Account (HSA) from Optum Bank can be paired with one of the high deductible health plans.

  • This account allows you to set aside pre-tax funds for future qualified healthcare expenses

  • Any unused funds in your account rollover year after year

Flexible Spending Account (FSA)

A Flexible Spending Account (FSA) through CBIZ Flex lets you set aside pre-tax dollars for qualified expenses. FSA funds are use it or lose it and must be spent by the end of the year calendar year.

  • Healthcare FSA – covers eligible medical, dental, and vision expenses (available only with a PPO plan).

  • Dependent Care FSA – covers eligible expenses like daycare (available to all employees regardless of medical plan).

  • Limited Purpose FSA – For those enrolled in a HDHP with HSA plan (for eligible dental and vision expenses only).

Life and AD&D

Life and AD&D insurance provides financial protection by offering benefits in the event of death or dismemberment. Basic Life and AD&D is paid by your employer and includes 1x your annual salary up to a maximum of $100,000. You may also purchase additional Voluntary Life and AD&D insurance for you and your dependents.

Disability

Disability insurance provides financial protection by offering benefits in the event of an inability to work due to injury or illness. Short-Term Disability (STD) covers 60% of your weekly earnings up to $4,000. Long-Term Disability (LTD) covers 60% of your monthly earnings up to $15,000 per month. Both insurances are provided by your employer at no cost to you!

Additional Benefits

Employee-paid additional benefits are available to help offset costs during an emergency.

  • Accident Insurance – Provides cash benefits to help cover unexpected costs, such as medical bills or lost income, after a covered accident.

  • Hospital Insurance – Pays cash benefits if you’re admitted to the hospital, helping with expenses your health plan may not cover.

  • Critical Illness Insurance – Offers a lump-sum payment if you’re diagnosed with a serious condition like cancer, heart attack, or stroke.

Legal and Identity Theft Benefit

This plan administered by MetLife provides you with unlimited access to attorneys for services like wills, contracts, real estate, and identity theft, all for a simple payroll-deducted monthly premium.
