MEDICAL

First Medical Plan

$100/$300

Deductible Plan

This plan offers a $100 individual and $300 family deductible. With this plan, preventive care is covered 100% and you will have set copays for doctor’s appointments, urgent care and ER visits. This medical plan also includes your pharmacy coverage.

First Medical Dental Plan

  • There is a no deductible

  • Preventive services are covered 100% for pediatric and 20% of the contracted rate for adults

  • Annual Benefit Maximum of $500

  • No orthodontic coverage

  • Must be enrolled in medical coverage to enroll in the dental plan

First Medical Vision – Vision International Network

  • $0 copay for in-network eye exams

  • $15 copay for first pair of lenses and $25 copay for second pair of lenses (every 12 months) AND

  • $15 copay for first frame and $25 copay for second frame (every 12 months) OR

  • $30 copay for contact lenses (every 12 months)

  • Must be enrolled in medical coverage to enroll in vision plan

Life and AD&D

Life and AD&D insurance provides financial protection by offering benefits in the event of death or dismemberment. Your employer provides you with a Basic Life and AD&D insurance benefit of $100,000 at no cost to you. You may also purchase additional Voluntary Life and AD&D insurance for you and your dependents.

Disability

Disability insurance provides financial protection by offering benefits in the event of an inability to work due to injury or illness. Short-Term Disability (STD) covers 60% of your weekly earnings up to $4,000 per week. Long-Term Disability (LTD) covers 60% of your monthly earnings up to $15,000 per month. Both of these benefits are provided by your employer at no cost to you!

Voluntary Benefits

Employee-paid additional benefits are available to help offset costs during an emergency.

  • Accident Insurance – Provides cash benefits to help cover unexpected costs after an accident, such as medical bills or lost income.
  • Hospital Insurance – Pays cash benefits if you’re admitted to the hospital, helping with expenses your health plan may not cover.

  • Critical Illness Insurance – Offers a lump-sum payment if you’re diagnosed with a serious condition like cancer, heart attack, or stroke.

Legal and Identity Theft Benefit

This plan administered by MetLife provides you with unlimited access to attorneys for services like wills, contracts, real estate, and identity theft, all for a simple payroll-deducted monthly premium.

Employee Assistance Program (EAP)

Your Employee Assistance Program (EAP) is a free benefit available to you and your family, offering resources to support your mental well-being. As part of this program, you have access to up to five counseling sessions each year at no cost.
